Abstract

The Registered Nurses Association of Ontario (RNAO), with support from the Ministry of Health and Long-Term Care, has embarked on a project of development, pilot implementation, evaluation and dissemination of best practice guidelines for nurses in Ontario. This project has been instrumental in facilitating a partnership between an acute care community hospital and two long-term care facilities in northwestern Toronto - Villa Colombo and Central Park Lodge and the Keele-Lawrence Centre. This article will outline the challenges as experienced in the acute care community hospital in developing partnerships not just with the two long-term care facilities but also within the organization. This article will also offer in-sight into the strategies used to overcome the challenges and to support the implementation of the guideline at point of care.
