Abstract
Cultural change is the first step in addressing the issue of patient safety. Strategies to achieve this must be well thought-out and extremely well-executed. However, action may be constrained by tension between federal and provincial governments. The provincial licensing bodies must be involved in rethinking the approach to error. Local leadership stimulated by a national agenda is the best way to proceed.
