The Future Starts at Home: Reimagining Care in Ontario

Home and community care in Ontario is delivered by a network of partners and increasingly connected teams with a shared purpose to serve a growing number of patients and caregivers in their homes in every part of the province.  Every day, caregivers and patients rely on care plans that include a combination of nursing, personal supports, occupational and physical therapies, and a variety of community-based health and social services, to remain in their communities while they receive care.
