CQCO Quality and Innovations Awards Call for Nominations

 The Cancer Quality Council of Ontario (CQCO), in partnership with Cancer Care Ontario (CCO), is pleased to announce a call for applications for its annual Quality and Innovations Awards. The Quality and Innovations Awards, co-sponsored by the Canadian Cancer Society – Ontario division, is a yearly competition which recognizes significant contributions to innovation in the delivery and quality of cancer care. The Awards were established to encourage and reward the development of innovative or quality initiatives aimed at enhancing and improving the performance of cancer care in Ontario.

This year, the Quality and Innovations Awards will place particular emphasis on primary care integration.  Applications highlighting quality or innovative initiatives which facilitate efficient and effective transitions in care are strongly encouraged. These may include activities or initiatives that are designed to increase communication between/among sectors to improve the quality of care experienced by patients or; collaborative efforts among service providers designed using a holistic approach based on the continuum of care for cancer patients.

Health care organizations that have developed and implemented quality or innovative initiatives related to one or more of our seven (7) quality domains are encouraged to submit a full application by Friday August 13, 2012. The successful applicant will be acknowledged at our Quality and Innovations Awards ceremony to be held on Thursday November 29, 2012 from5:30-7:00 pm.
