At Unity Health Toronto, clinicians, scientists, leaders and teams are using research, innovation and partnership to break down siloes and advance care beyond their walls. Purpose-built data sharing networks are advancing quality improvement and research projects across the province, while local partnerships are improving information sharing and helping patients move seamlessly from site to site. With remote robotics, world-class surgeons in Toronto are bringing specialized brain surgery to rural Canadians, while a home-grown research program serving people experiencing homelessness is being replicated in other Canadian cities.
Unity Health knows that no single system can change on its own. This session, delivered by Unity Health President and CEO Altaf Stationwala, will explore how the organization is creating a collaborative ecosystem where people are empowered to build infrastructure and programs that improve access to and delivery of high-quality patient care.

Home and community care in Ontario is delivered by a network of partners and increasingly connected teams with a shared purpose to serve a growing number of patients and caregivers in their homes in every part of the province. Every day, caregivers and patients rely on care plans that include a combination of nursing, personal supports, occupational and physical therapies, and a variety of community-based health and social services, to remain in their communities while they receive care.
