Abstract

One of our goals in writing this paper was to further the emerging Canadian discussion on patient safety. The responses and criticisms in the commentaries suggest that the discussion has started. We appreciate the insights contained in these comm-entaries, and thank the authors for their careful reading of our work and their thoughtful comments. In this reply, we would like to highlight some key issues and suggest some additional steps that will further the safety agenda here in Canada.
