Have you ever had trouble finding health and social services in your community that meet your needs, or those of your clients or patients?
The People, Health Equity and Innovation (PHI) Group at the University Health Network is developing a web-based tool to help seniors, their family caregivers, and health professionals find health and social services in their communities. To gather design requirements for this tool, they are interviewing people to explore their experiences searching for health and social services, and their perspectives on tools that could help. They are seeking patients over the age of 65, living with one or more chronic conditions, and who use the internet, and family members, who are involved in caregiving for someone over the age of 65 and living with one or more chronic conditions, and who use the internet.
