Abstract
Project Background
The Canadian Council on Health Services Accreditation (CCHSA) released its inaugural Patient Safety Goals and Required Organizational Practices to its member organizations and surveyors in December 2004. CCHSA's first patient safety goal is to "Create a culture of safety within the organization." To help support a culture of safety, organizations need to have an understanding of staff and physician perceptions of the current state of client/patient safety culture.
